The organization combines social and clinical care with local outreach to improve access and patient experience, offering same and next-day virtual appointments with a doctor for preventive or urgent care. Each member is supported by a dedicated care team including a promotora de salud, a virtual physician, and a virtual behavioral health specialist to create tailored, holistic care plans. With a focus on care coordination and lasting community connections, Zócalo Health operates across multiple U.S. locales to serve families and underserved populations.
