Two Conduction-System Pacing RCTs Give Conflicting Results
Two randomized trials comparing conduction‑system pacing (CSP) with traditional biventricular (BiV) cardiac resynchronization therapy produced opposite results. The Chinese HeartSync‑LBBP study found that left‑bundle branch pacing reduced the composite of death or heart‑failure hospitalization and improved ventricular remodeling, while the Brazilian PhysioSync‑HF trial failed to meet non‑inferiority, showing higher mortality and less LVEF gain with CSP. A key difference was operator experience, with all HeartSync procedures performed by high‑volume implanters versus many low‑volume operators in PhysioSync. The findings suggest CSP should remain limited to expert centers pending larger trials.
