Researchers analyzing data from 11,319 participants across four major cohorts found that elevated lipoprotein(a) levels (>50 mg/dL) and a coronary artery calcium (CAC) score above zero each independently increase the risk of atherosclerotic cardiovascular disease (ASCVD). The combination of high Lp(a) and any detectable CAC amplified risk, with the greatest hazard observed in individuals with CAC ≥300 and elevated Lp(a) (hazard ratio ≈ 6). Among middle‑aged men, the absolute event rates were highest, while a CAC score of zero kept long‑term risk low even when Lp(a) was elevated. The findings suggest CAC imaging can refine risk stratification and guide preventive therapy decisions for patients with high Lp(a).
