
Medical Debt Drives Suicide, Not Patient Non‑Compliance
Sixteen percent of suicides in the United States have medical debt as a contributing factor. We talk about patient non-compliance in medicine as if it is a behavioral problem. For a significant number of patients, it is a financial one. They are not ignoring your treatment plan. They cannot afford it. And many of them will never tell you. Adam Cunningham, a health care executive, describes this dynamic on #ThePodcastbyKevinMD. He shares the story of a friend in Maine who lost her job as a women's advocate due to budget cuts, then lost her employer-tied health insurance. She had developed aggressive rheumatoid arthritis. While she still had coverage, her insurance company delayed and complicated the process long enough that she received no real treatment before the plan ended. After that, she had nothing. She could barely function. What happened next challenges assumptions about where quality health care can be found. She got biologic treatment at a JCI-accredited hospital in China. JCI, or Joint Commission International, is the same accreditation standard applied to top hospitals in the U.S. The cost was dramatically lower. Within four months, she was back to working and living a functional life. Cunningham is not arguing that every patient should leave the country for treatment. He is making a narrower, harder point: when the same treatment costs hundreds of thousands of dollars in the U.S. and thousands elsewhere at comparable quality, patients are going to make desperate calculations. Some of those calculations end in suicide. That is not a hypothetical. It is a documented pattern. His challenge to clinicians is direct. He says the most important question a practitioner can ask is whether the patient can actually afford the treatment being recommended. Not what insurance says. What the patient can actually do. He has watched patients agree to a prescription in the exam room and then never fill it because the cost is something they are too embarrassed to disclose. This is the uncomfortable part for anyone in clinical practice: we document non-adherence without investigating whether the barrier is financial. We treat the symptom of non-compliance without diagnosing the cause. Cunningham also raises a practical first step that most patients overlook. Before considering treatment abroad, speak with a patient advocate. He describes the hospital's responsibility as making money and the advocate's responsibility as helping the patient.
