Across 381 patients and 90,996 care‑manager minutes, only 56% of activity met billing criteria, yielding an optimal reimbursement of $91.61 per hour. More restrictive billing scenarios lowered the rate to $79.32, while Medicaid‑level payments could fall to $39‑43 per hour. The study highlights a gap between clinical need and billable services for complex patient populations.
